You may download the necessary new
patient forms here. You can print these forms, fill them out, and then bring
them to your first visit.
All new patients should fill out the Client Information Form.
If you intend to address weight loss or smoking issues, please also complete the
appropriate Weight Release or Stop Smoking form as well.
To download a form, right-click (ctrl-click for Mac) and choose Save As... from
the menu. Choose where to save the file and click Save. Forms are provided in
both PDF and DOC formats.
